Patient Information & Forms
Please review the below in preparation for your upcoming first appointment. We look forward to meeting with you.
Location: We are located in a two-story brick building with large green awnings. Subway and Supercuts are on the first floor. Walk to the left-hand side of the building for a separate entrance to the second floor offices. There is plenty of free parking around the building.
Form: Please download and fill out the Medical History form prior to your first appointment and bring with you. You may also fill them out at our clinic, just arrive early to do so. Download here and, if possible, fax (781-209-7301) or email back to email@example.com in advance of your initial appointment.
Clothing: Please bring a pair of shorts or loose clothing for postural assessment at your first visit.
When you arrive: If our office manager is not at the front desk, please have a seat and your therapist will be out to greet you shortly.
Cancellation Policy: In the event that you need to cancel an appointment, we appreciate your consideration of our 24-hour cancellation policy. Any cancellation with less than 24 hours’ notice will be responsible for the full payment. With short notice it is difficult to fill an open appointment, and we have many patients on our waitlist. Thank you for your understanding & consideration.
Online booking: You can book online through our website (See “Appointments” page) or via the MindBody App. If you are an existing patient, use the email address to which you receive appointment confirmations. You already have an account, you just need to create a password. When booking online, you must pay for your appointment to reserve your time slot. Appointments are not booked unless you receive an email confirmation.

Select your Service: Are you looking for NeuroMuscular Therapy, Therapeutic Massage or Corrective Movement?
Choose your desired session length
Choose which therapist you would like to search for or choose "Any Staff" to find any next availability
Orange dots indicate available appointment times - click on one of those and choose your desired time
Click "View Pricing"
Click "Book and Buy" ~ You do have to purchase the appointment in advance to secure your spot
